Cervical Spine Platelet-Rich Plasma and Isometric Strength Synergistically Improve Disability in Chronic Whiplash Associated Disorders: A Series of Mediation Analyses

What question is this study trying to answer?

This study investigates whether rehabilitation following cervical facet joint platelet-rich plasma (PRP) injections improves clinical outcomes, specifically cervical range of motion (ROM) and isometric strength, and whether these physical improvements mediate reductions in pain and disability in patients with chronic whiplash-associated disorder (WAD).

What did we learn?

In patients reporting successful outcomes after PRP treatment, rehabilitation over three months resulted in clinically significant improvements in neck disability, isometric strength, and cervical ROM. Pain reduction after PRP directly decreased disability independent of changes in strength or ROM. Meanwhile, improvements in strength independently contributed to reduced disability, but ROM did not mediate disability changes. These findings suggest a synergistic effect where pain relief from PRP combined with strengthening exercises enhances functional recovery.

Why is this relevant?

Chronic neck pain from cervical facet joint injury after whiplash is difficult to treat. PRP is emerging as a promising biologic treatment due to its anti-inflammatory and tissue healing properties, but the role of rehabilitation alongside PRP remains unclear. This study provides evidence that rehabilitation focused on strength improves disability outcomes beyond pain relief alone. Optimizing combined treatment strategies can enhance recovery in patients with chronic facet-mediated neck pain.
