This study evaluates the effectiveness of lumbar medial branch radiofrequency ablation (LMBRFA) performed with a multi-tined cannula using a perpendicular approach (P-LMBRFA) in patients with confirmed facet joint-mediated low back pain.
At 3 months post-treatment, about half of the patients experienced significant improvements in pain (≥50% reduction on NRS) and disability/quality of life (≥17-point improvement on PDQQ-S). For those whose symptoms eventually returned, the average duration of relief was nearly 9 months with high levels of retrospective pain relief. Regular exercise and active employment were associated with better outcomes. These results suggest the perpendicular multi-tined cannula approach can provide pain relief and functional improvement comparable to the traditional parallel monopolar technique.
Low back pain due to facet joint pathology is common and often disabling. While radiofrequency ablation is an established treatment, the traditional parallel monopolar technique can be challenging due to anatomical variability. The perpendicular approach with multi-tined cannulae simplifies the procedure, may overcome technical limitations, and shows promise as an effective alternative. This study adds valuable clinical evidence supporting the perpendicular multi-tined technique’s real-world use and highlights patient factors that may influence success.
